ABSTRACT

Gardner–Diamond syndrome (GDS), or autoerythrocyte sensitisation, is a rare cause of recurrentpainful bruising with a typical anamnesis and prodrome. We describe a patient with GDS and discussthe literature surrounding this unique condition.

ABSTRACT

Las equimosis cutáneas representan la más frecuente de las lesiones encontradas en casos de agresión física.Se producen por de la ruptura de vasos sanguíneos (principalmente capilares), que conduce a extravasación de la sangre y acumulo en el tejido intradérmico, subcutáneo o en ambos. Posterior a esto se inicia un proceso inflamatorio local que conlleva a la degradación de la hemoglobina en subproductos cromóforos (hemosiderina, biliverdina, bilirrubina, metahemoglobina) que dan la coloración inicial y sus cambios. El método visual usado durante décadas es poco efectivo y lo más recomendable es la evaluación las lesiones con respecto a la aparición de la coloración amarillenta. Se están desarrollando nuevos métodos para mejorar la datación de las lesiones, que parecen bastante prometedores (espectrofotometría y resonancia magnética).


Skin bruising are the most frequent lesions found in cases of physical aggression. Product of blood vessels rupture (especially capillaries), leading to extravasation and accumulation of blood in the intradermic tissue, subcutaneous tissue or both. After that, begins a local inflammatory process leading to the degradation of hemoglobin chromophore subproducts (hemosiderin, biliverdin, bilirubin, methemoglobin) which give the initial coloring and changes over the time. The visual method used for decades is ineffective and the best approach is looking por the presence of the color yellow into the bruise. At present, new methods are developing to improve the dating of injuries that seem quite promising (spectrophotometry and magnetic resonance).

ABSTRACT

A 26-year-old male presented to us with defective vision in the left eye. He had best corrected visual acuity (BCVA) of hand movement (HM) in right eye and 6/9 in left eye. He had ptosis with ectropion in both eyes and relative afferent pupillary defect (RAPD) in right eye. Intraocular pressure (IOP) was 46 and 44 mmHg in right and left eye, respectively. Fundus showed glaucomatous optic atrophy (GOA) in right eye and cup disc ratio (CDR) of 0.75 with bipolar rim thinning in left eye. Systemic examination showed hyperextensible skin and joints, acrogeria, hypodontia, high arched palate, and varicose veins. He gave history of easy bruising and tendency to fall and history of intestinal rupture 5 years ago for which he had undergone surgery. He was diagnosed as a case of Type IV Ehler-Danlos syndrome (EDS) with open angle glaucoma. He underwent trabeculectomy in both eyes. This is a rare case that shows glaucoma in a patient of EDS Type IV. Very few such cases have been reported in literature.

ABSTRACT

Aims: To define the nature of non-radiographically evident injuries of the ankle and hindfoot in children who exhibited persistent pain, swelling and refusal to bear weight after 6 weeks of immobilisation. Methods: Ten children (7 boys, 3 girls; age range 5 to10 years) were included in the study. They presented with normal plain radiographs after an acute traumatic ankle injury. They were all immobilised for 6 weeks. The detection of residual symptoms and signs indicated the need of a limited magnetic resonance imaging (MRI). It revealed bone bruising localized to the hindfoot in all cases. Bone bruises were classified according to anatomical location and whether they were solitary lesions or were combined with other soft tissue injuries. Results: From a total of 10 cases, 6 were unifocal, 3 of them were localized to the talus and 3 to the calcaneus, while in 4 multifocal cases bone bruising involved all the bones of the hindfoot. All cases were associated with a varying degree of soft tissue injuries including signs of ankle and subtalar joint effusion, but there was no evidence of ligamentous injuries in our patients. The total period of immobilisation reached 3 months for unifocal bone bruises and 4 months for multifocal lesions. In all cases, a prolonged recovery period exceeding 2 months followed before return to normal activities including sports. The long-term follow-up ranged from 2 to 5 years. No complications were detected. Conclusion: A limited MRI examination is indicated in children with residual symptoms following acute ankle injuries. Bone bruising localized to the hindfoot was the “occult” bone lesion diagnosed in all our cases. This study indicates that bone bruising or contusion should be classified as a distinct clinical entity best visualized on MRI with fat suppression

El trauma es la principal causa de muerte en los pacientes pediátricos mayores de 1 año, siendo el trauma abdominal responsable del 10 por ciento de las causas de muerte. Las lesiones de vísceras huecas es inferior al 1 por ciento , sin embargo, su mortalidad es del 20 por ciento en el caso de perforación intestinal. La tomografía computada es el método de elección en la identificación y cuantificación de las lesiones en trauma abdominal, dado su excelente rendimiento para lesiones de vísceras sólidas, con menor sensibilidad en lesiones de vísceras huecas, por lo que en estas últimas es importante una alta sospecha clínica y análisis minucioso de las imágenes por parte de los radiólogos. Se realizó una revisión retrospectiva de los hallazgos en pacientes pediátricos referidos a tomografía computarizada, con historia de trauma abdominal contuso. Los principales hallazgos en tomografía computada sugerentes de lesión de víscera hueca fueron: aire extraluminal, extravasación de medio de contraste, presencia de líquido libre intraperitoneal, coágulo centinela adyacente al asa comprometida y engrosamiento de la pared intestinal.


Trauma is the leading cause of death in pediatric patients older than 1 year, with abdominal trauma accounting for 10% of causes of death. Hollow viscera injuries are less than 1%, however its mortality is 20% in the case of intestinal perforation. Computed tomography is the method of choice for the identification and quantification of abdominal trauma injuries, given its excellent performance for solid viscera injuries, with less sensitivity in hollow visceral injuries, so that in the latter a high clinical suspicion and thorough analysis of the images by radiologists is important. A retrospective review was conducted of the findings in pediatric patients, with a history of blunt abdominal trauma, referred to computed tomography. The main findings on computed tomography suggestive of hollow visceral injury were: extraluminal air, extravasation of contrast medium, the presence of free intraperitoneal fluid, sentinel clot adjacent to the affected loop and thickening of the bowel wall.

ABSTRACT

El Arnica montana es uno de los medicamentos que se emplean con mayor frecuencia en la práctica homeopática. Comúnmente, es la primera elección en la atención de traumatismos, contusiones y golpes, especialmente de partes blandas. También se emplea en torceduras, fracturas, esfuerzos exagerados, enfermedades reumáticas, traumas emocionales y para mejorar la recuperación de los pacientes que se someten a intervenciones quirúrgicas. Debido al interés que genera, el Arnica montana ha sido objeto de numerosos estudios en diferentes áreas para comprobar su alcance y determinar sus mecanismos de acción. A continuación se presenta una revisión de algunos artículos publicados sobre este medicamento; unos versan sobre sus principios activos y otros analizan diversos experimentos que se han efectuado.


Arnica montana is one of the most often used drugs in homeopathic practice. Commonly, it is the first choice in the trauma care, bruises and bumps, especially soft tissue. Also used in sprains, fractures, exaggerated efforts, rheumatic diseases and emotional trauma to improve recovery of patients who undergo surgery. Due to the interest generated, Arnica montana has been the subject of numerous studies in different areas to check their range and determine their mechanisms of action. Below is a review of some articles published on this medication, some deal with other active principles and discusses various experiments that have been conducted.

O trabalho teve como objetivo avaliar a influência do transporte na ocorrência de lesões, em carcaças de bovinos abatidos no Pantanal Sul Matogrossense, por meio da quantificação do número e do tamanho das lesões, assim como a localização da freqüência de ocorrência das lesões nos principais cortes comerciais padronizados para o mercado interno. Do total de 121 carcaças avaliadas, foi observado que 102 (84,3 por cento) tiveram uma ou mais lesões, totalizando 270 lesões que resultaram na remoção de 56,1kg de carne, com média geral de 0,5kg por animal ou 0,6kg por animal, considerando-se apenas os animais que tiveram lesões. A freqüência de lesões em carcaças de bovinos evidenciou diferença significativa de acordo com as condições de transporte. As maiores proporções de lesões foram encontradas em animais submetidos ao transporte rodoviário por mais de uma hora e distâncias maiores que 70km, sendo grande parte em estradas não-pavimentadas. Conclui-se que os animais submetidos à série sucessiva de manejo e transportados em estradas não-pavimentadas por longas distâncias apresentaram maior proporção de lesões, sendo que os animais provenientes do lote VI apresentaram maior número de lesões.


This study aimed to assess the influence of transport on the occurrence of carcass bruising of cattle slaughtered in the Pantanal, by quantifying the number and size of bruises, locating the frequency of occurrence of bruises in the main commercial cuts standardized for interne market. From a total of 121 carcasses assessed, 102 (84.3 percent) had one or more bruises, totaling 270 bruises, which resulted in the removal of 56.1kg meat. The frequency of carcass bruises of cattle showed significant difference, according to the conditions of transport. The highest proportions of bruises found in animals submitted to road transport for more than one hour in distances longer than 70km, a great part of which on dirty toads. The conclusion is that the animals submitted to the successive series of handling and transported on dirty roads for long distances caused the highest proportion of bruises, whereas the animals submitted to lot IV presented the highest number of bruises.

ABSTRACT

Autoerythrocyte sensitization syndrome (Gardner-Diamond syndrome), also known as painful bruising syndrome or psychogenic purpura, is characterized by sudden and spontaneous painful bruising and swelling on both extremities, and is a psychologically induced painful bruising condition. The exact etiology of the syndrome is an enigma, even today. It occurs primarily in young to middle-aged women who usually have psychogenic disorders. A diagnosis of this syndrome is made on the basis of the clinical history, dermatological examination complemented by a positive autoerythrocyte sensitization test, psychiatric evaluation and the absence of any organic cause. We present a 20-year-old man with characteristic features of this syndrome, who had a positive autoerythrocyte sensitization test. To our knowledge, this is the first case in an adult male reported in the Korean literature.

ABSTRACT

Autoerythrocyte sensitization syndrome is characterized by sudden and spontaneous painful bruising and swelling occurring in adult female patients with emotional disorders. Systemic symptoms often accompany the onset of these lesions. A diagnosis of this syndrome can be made in a patient who has the typical history and clinical picture of the syndrome and in whom a skin test with the patient's blood reveals a positive reaction.
